To best serve the children and youth participating in our RE classes and programs, we ask that parents fill out registration and other permission forms. This helps us serve the needs and protect the privacy of both you and your children. Thank you!
Below you will find RE registration forms for infants, toddlers, children, and youth, as well as other forms we require for special activities, for Visitors to RE Classes, and for our Coming of Age (7th and 8th grade age youth) and Our Whole Lives sexuality education programs (various age groups). Child and Toddler RE Registration Form – To register a child (pre-K and 6th grade) or an infant or toddler, please download this fillable PDF form, fill it out, and email it to email@example.com or drop it in Michael’s mailbox at the UUFC.
